A NEW RESTORATIVE RESECTION OF LOW RECTAL CANCER : ABDOMINO-PREANAL EXTRASPHINCTERIC RESECTION OF RECTUM.


Abstract
Various anus-preserving procedure in rectal cancer resection has two main problems. One is deep opereation field for radical resection and anastomosis. The other is preservation of defecational continence. The authors have new idea which can
easily
resolve the two problems simultaneously. It is abdominopreanal extrasphincteric resection of low rectal cancer(Oah's rectal Resection). Abdominal approach to intraabdominal pelvic dissection was performed, and then preanal extrasphincteric
approach
between anus and genital organs was performed by the authors for resection of retal cancer, which was located at from 3 cm to 5 cm proximal from anal verge. This operation was performed in 15 cases of low rectal cancer for last five years at our
hospital.
@ES The results obtained were as follows :
@EN 1) Preanal extrasphincteric approach in 15 cases of low rectal cancer was easily maden under guidance of perineopelvic guider and it provide good drainage pathway in cases of coloanal anastomoic leakage resulting little pelvic sepsis
2) Distal surgical margin of rectum was cutted and sutured easily under direct observation of the lesion.
3) Postoperative fecal continence was good in 11 cases(73.3%) according to Kelly's clinical assessment. (i.e. wide operation field, adequate distal resection, and good anal function.)
